You have some other discs to try with that doesn't have the same media code (the red text)?

How did you manage to install an OLDER firmware?!

 

You want TN03.

Seriously, are you 110% sure that's a new disc? It still doesn't look in the least bit 'new'.

 

Take another disc from the middle of your new spindle of discs and do it again - remember, you aren't attempting to burn anything, just copy+paste that disc info again. (after you're on TN03 firmware)

Can you buy some different discs then?

 

Your drive isn't initialising the 'SONY16D1' ones properly.

 

I was hoping see to the disc information of another new disc once you'd updated to the TN03 firmware and BEFORE you'd attempted to write/burn anything onto it. Can you do that for me please?

...and now we see the real error - your drive is reporting a 'Write Error' when trying to burn the media.

 

Have you tried burning at 8x, 16x and 18x too as suggested earlier?

 

Have you tried cleaning your drive with a cleaning disc?

 

You really do need to buy some different media (with a different MID/dye) if you want to rule that out as the cause of the problem.
