I/O Read Error

Hey when I tried creating a .iso file from a disc, it says "I/O Error! Interpretation: Check condition" and it had trouble reading the disc. (The disc is an original, wasn't copied or anything) I tried with 8x, then 2x, then 1x but they all failed. I'm using a Philips DVD+-RW DVD 8631 drive. I also changed the device 0 to PIO and device 1 to DMA. I restarted the computer after I applied the new settings but it still doesn't work. Any ideas?

P.S. When the disc loads, it says the label and other stuff is unknown and it says the size is 0 MB, even though the disc is like 4.34 GB and it's an original Wii disc, Call of Duty Black Ops.
