No Medium, I/O Error

I know this is probably covered a couple times, however I could not remember where I read it, nor could I find it again. So any help would be greatly appreciated. I have two questions. One when I place a Verbatim DL +R in my drive it says no medium present. How do I fix this? Second when I use my other discs, Memorex DL +R I can burn until I receive this error message, in the log below. Please help?

Update your burner's firmware (remove any disc and close the tray before updating then reboot after it's finished) and try again with the Verbatim blanks. If you still get the "medium not present" message then throw that burner in the garbage and treat yourself to a new burner because you won't find better quality blanks than Verbatim.
