I 21:53:51 Destination Device: [0:0:0] MATSHITA BD-MLT UJ-220S 1.04 (F:) (ATA)

I 21:53:51 Destination Media Type: DVD+R (Disc ID: CMC MAG-E01-00) (Speeds: 2.4x, 4x, 6x, 8x)

What a "killer" combination you have there. :)

Matshita are famous for their bad burners in combination with the cheap/worst media known.

Try all supported speeds, start with 4x up as you ruled out 2,4x

You can try cleaning the lens with a cleaning disc or manual if you know how to do it.

Use high quality discs. Verbatin and Tayio Yuden are recommended.
