Strange Error at 50% and ISO region settings

When trying to create an ISO from a dual layer DVD I get the following error:

This happens at exaclty 50% progress. Anybody got any ideas?

 

I also noticed that it looks like its creating the ISO in NTFS format when the disk itself is PAL. Any reason for this or way to change the settings?
