2 things:

 

1. Get Verbatim, +R, 2.4x, MKM-001-00, Made in Singapore media and throw your current media away.

 

2. Update your firmware - http://forum.rpc1.org/dl_firmware.php?download_id=2005

 

As well, your burner is getting very long in the tooth. Keep an eye on it.

It was for my NEC drive. Downloaded the update and double clicked on it and it flashed itself. It was an official firmware release and not one some bloke had made.
