I 21:17:36 Destination Media Type: DVD+R (Disc ID: MKM-003-00) (Speeds: 4x, 6x, 8x)

I 21:17:36 Destination Media Sectors: 16,384,000

Upgrade your burner's firmware to the latest version SB03 and try again. Remove any disc and close the tray before updating the firmware. Reboot after it has finished.

The first error you get is the real one, the ones from retries don't always mean anything.

So in your case, this is the real error...

W 21:28:38 Failed to Write Sectors 2537792 - 2537823 - Reason: Write Error

Basically, your drive is saying it can't write to the media (for whatever reason).

Hopefully the SB03 firmware mmalves mentioned will sort it out, but if it doesn't you should try to find some MKM-001-00 verbatims instead (the 2.4x ones).
