Constantly getting "Logical Block Address out of Range"

Over the last 10 disks I've tried to burn, 9 of them have been coasters due to this happening. I'm using Memorex disks at max speed. I've been burning disks for years and have never had this happen, and I almost exclusively use a combination of Verbatim and Memorex DVD+R(DL)s and CD+Rs, always burnt at max speed. The DVD drive I'm using is a Samsung SH-S223F that was bought from Newegg about 1.5 years ago. In the past I've gotten errors occasionally that I think were something like "Invalid address for write," but they were nowhere near as often as this new error. Can anyone help me out on why this is happening? I've attached a log from this past time that it errored.

Even if you have gotten Memorex DVD+R DL's to burn in the past, I would recommend you check the Manufacturer ID on them. If it says CMC, avoid using those.

If you're getting LBA errors on the Verbatims, then, it may be your drive needs replacing. My experience has been my burners need replacing about every 2 years. But, I work my drives to death, pretty much. :D

I also get mine from NewEgg. :)
