BD-R DL Fails when moving from layer 0 to layer 1

I have a Pioneer BD-RW BDR-206D and am trying to burn a dual layer disc using Optical Quantum BD-R DL 6x media.  This is my first try at burning BD DL discs, so I'm hoping there's something simple I missed.  I just used all the default options, but it looks like there was an error when the burn went from layer 0 to layer 1.  See the log.  I'd appreciate any suggestions!
