Transfer multiple HD partitions to a new HD

I need to move two partitions from one hard disk to another. One is c:, the system partition; one is d:, the data partition. Both are full and I have another hard disk ready for them to go to. I was told imgburn can image the two partitions to a single iso, but can't figure out how to do this.
