I am having 100% failure rate for some reason.

If you've tried different media and are on the latest firmware, your drive is probably on its way out.

 

Try cleaning the lens and perhaps think about returning the defective drive or buying a new one.

Changed Media to Verbatim+R and got a 4% burn.

Ifcrule1972 I have been remiss in not thanking you for the references pertaining to my 4164B which truly is a piece of junk. I soldiered on with it since your post but now it appears to have finally given up the ghost. Last night I spent a great deal of time with Dell getting them to replace a faulty monitor. Now I will have to go back to the well once more. Many thanks.

You simply got a bad one, or a bad controller or something. They are an excellent drive.
