Update firmware on both your drives
writer D400 http://www.firmwarehq.com/Samsung/TS-H653F/files.html
reader D700 http://www.firmwarehq.com/Samsung/TS-H353B/files.html
Use quality discs from Verbatim.
Try different speeds available.

Anyway, I'm not changing it again so you'll just have to shut your eyes when you look at it.
I just had a good heartfelt laugh

Don't worry about image file formats. ImgBurn will make the appropriate format for the disc content. Let it choose what it wants.
Most of the times it will be ISO and sometimes will be CUE/BIN.
The MDS file type is primarily associated with 'Media Descriptor'. This file is used by several different programs. It generally is found with an .ISO CD image file and is the equivalent of what a .CUE file is to a .BIN image.
-
Use Virtual CloneDrive to mount the iso than recreate the iso with ImgBurn containing your modifications.
I'm afraid this is most of you can get from what you ask.

It's not possible to booktype DVD-R, only DVD+R (SL and DL).
Put the settings back and just burn the DVD's there will be OK.
The DVD-R (minus) is more compatible to older players than DVD+R (plus), that's why probably you are confusing things.

You mean this Guide? http://forum.imgburn.com/index.php?showtopic=11190
That guide is for when you want to slipstream SP's and integrate the updates in the installation disc. It's pretty self explaining, what you need to do if you need it.
-
You don't need to extract anything from the iso.
Use Write image file to disc, select the iso as source, put a blank disc in the drive and hit Burn button.
-
what if i change to verbatim and it still fails?a new drive? even tho its only a year? imma try burning a new game first.
When I said buy a new drive, I just answerd your question.
Meaning if you try Verbatim and still fail, replace burner.
A drive that can't burn propperly a Verbatim disc it is pretty useles, you know.
-
Memtest ISO images are already bootable so you need to use Write image file to disc.
-
Go with a higher speed 6x or 8x, forget about the lower speeds fairytale.
If Verbatin don't go, buy a new drive.

Try this to view hidden files http://pcsupport.about.com/od/windows7/ht/show-hidden-files-windows-7.htm
What was the reason for not leting you to save to D:\ partition? There is no space available or is it FAT32 formated?
If its FAT32, you can save the project as Hard Disk Folder instead of ISO by changing the Target device settings in DVD Shrink.
Than use this guide to burn the files to a new DVD http://forum.imgburn.com/index.php?showtopic=4632

DVD Flick comes with an old version (2.4.4.0) wich resides in Program Files\DVD Flick\imgburn folder.
You can replace it with the new one that you installed in Program Files\ImgBurn.
-
The firmware update will improve media support of the drive.
Can't you save the iso to other partition instead of C:\ ? As I said, Windows 7 will not let you save to C:\ unless you modify the permissions and run the program as administrator.
I still think the ISO is somewhere in C:\Users\YOUR_USER\Documents but probably it's hidden, that's why you can't find it by search and Windows asks to overwrite it.
-
In Windows 7 you don't have the permission to save on C:\ partition by default.
It will be saved in C:\Users\YOUR_USER\Documents though. See if you can find it there.
Alternatively you can track the path by starting again and look to the address bar where it will be saved when it tells you that already exists.
Btw there is a later firmware for your dive http://www.firmwarehq.com/LG/GH40L/files.html

TSST stands for Toshiba Samsung Storage Technology and is presented under Samsung branded drives on FirmwareHQ.
Your specific model TS-H653Q has listed SC01 firmware update, but that is not matching your OEM version, your looks as 0303.
You can try find an update if it ever exist on your computer manufacturer site. Seems to be an HP laptop, am I right?
-
You can find old logs via Help menu.
Why would you use TAO anyway, it can cause problems on some players.
Try other speeds available.
Make sure you use the current version of ImgBurn.
-
Update firmware to latest 1.0B http://forum.imgburn.com/index.php?showtopic=17246
Also updated ImgBurn to current version.

The problem is that your drive is old (2006 is the latest firmware that's already installed) and don't like the new media, hence the only (2.4x) write speed supported shows, usualy there are more for that MID.
Cleaning the lens of the drive might help. Use a lens cleaning disc or manual.
Usualy Verbatim 2.4x (MKM-001-00) will decide if the burner is still alive, give it a shot.
If all fails Ricewind already recommended the solution for you.
-
Try again after installing the latest firmware http://www.firmwarehq.com/LG/GH22NS50/files.html
-
Just a hunch, couldn't be some antivirus or other protection software interfere?

You have no reason to do such noise. Did you read the pink box above before you posted?
No one will give you out, just be reasonable and describe your problem and we will try to help.

200 dvd's a month, my failures currently stand at 30 good, to 80 bad
Ha ha, curious. What math do you use?
p.s. As for the stars, just let them where just necessary not all over, we will now though why you put them anyway.
How to top up partly filed DVD+RWs?

There is no support for multisession.
You have to rebuild your disc with the additional files.
ImgBurn don't need space on hard drive if you burn to disc and uses little of the system memory.
