  1. Then you know what's screwing things up. Codec packs are rubbish, only install the codecs you need.
  2. Your drive is too old and probably doesn't comply with the MMC specs, which is why you're getting those weird values. Also I don't think it would do a good job burning newer media.
  3. If it's a DVD+RW disc then you need to do a Full Erase at least once before you can use it. After that you can just overwrite it (no need to quick erase).

  7. Try with other brands, or maybe with DVD-R, but sounds like your burner is defective. You could try a lens cleaning disc to see if it helps.

  12. Try burning at 6x or 8x with a brand new blank disc. You might also want to try cleaning the drive with a lens cleaning disc. If none of that solves your problem then you'll need to get a new burner (it's cheaper than a 10-pack of Verbatim blanks).

  14. You'll need dual-layer media (and a dual-layer capable burning) in order to burn that stuff as is. If you don't care losing some video quality then you could try DVD Shrink/similar programs in order to make it fit on the 4.7gb blank disc. By the way, if you want to just copy (i.e. not going to change anything) then you should be using Read mode to make a disc image then using Write mode to write that disc image onto media.

  16. The files won't be placed in the floppy image but on the data area of the disc, hence why the bootable floppy image must have CD-ROM support. Burn this ISO image to a disc (even a rewritable works) and make sure Verify is enabled. Boot with that disc, change to R: and your update files will be there. Tested it on a virtual machine and it's working OK.

You can get bootable floppy disk images in this website (may need to extract them with 7-Zip/WinRAR/etc), add the Adaptec update files in ImgBurn's Build mode, set the filesystem to ISO9660 only, set the floppy emulation parameters in Advanced -> Bootable Disc, then write the image/burn the disc. You can try on a rewritable or saving as an ISO for mounting on a virtual machine. As for the floppy disk, Win95/98 should be enough, as long as it has support for CD-ROM.

  19. Slim burners are rubbish so you need to use high quality media (e.g. Verbatim and Taiyo Yuden blanks) if you want to get good burns.

  23. Reinstalling Windows isn't what fixed your problem: you probably just reached a blank that wasn't as crappy the ones that failed before.
  24. You can try cleaning the lens with a cotton bud and rubbing alcohol, but be very careful. You already have the best media you can get on the market, so if cleaning doesn't solve the problem then you'll need to use another burner.
